Paradigm’s 12-month TTP program offers clinical and educational activities to support new nurses in their professional development and personal growth. Designed for highly motivated and dedicated graduates, the program provides structured, practice-based learning within a strong support framework, creating an enriching and nurturing environment. By fostering skill, compassion, and commitment, the TTP program helps new registered nurses successfully transition into field case manager roles, empowering them to make a meaningful impact on patients’ lives.
Our program is designed to provide support from your first day, through your entire career with Paradigm. All full-time case management hires who are recent graduates, or who have less than 12-months experience, are automatically enrolled in the Transition to Practice program.

A full-time case manager maintains a full-time caseload consistent with the complexity of cases. All cases require adherence to Paradigm or client handling requirements, including timely contact and reporting.
The ability to communicate effectively with a wide range of people on a daily basis requires high level verbal, written, and interpersonal communication skills.
Even the best planned out day can go awry so possessing strong flexibility, versatility, and critical thinking skills are of the utmost of importance for case managers.
Case managers at Paradigm use an advanced case management software solution to do their work. A laptop and cell phone are used daily, and case managers are expected to become fluent in all the tools needed to support their role.
Case managers do not provide direct patient care, but they rely on their strong clinical skills to assess and support the injured worker, communicate with providers, and evaluate care, all focused to improve the lives of injured workers.
