Wound Management

Chronic health conditions, including chronic wounds and urinary infections present a significant financial liability and usually are a management challenge for a customer.  These chronic conditions may develop within months of a catastrophic injury or may develop many years after the event.  Most individuals with a major wound are debilitated (sensory, motor or both) to some extent are not able to move body parts frequently enough to prevent pressure sores.  Aging with a disability presents the individual, payer and healthcare providers with a new set of problems and risks that are best managed with a coordinated care management approach.  Chronic wounds usually occur in individuals with a prior spinal cord injury, multiple trauma, burns or brain injuries (in a persistent vegetative state).

Treatment
Treatment includes a coordinated care approach to ensure the wounds heal appropriately and systems are in place to prevent re-occurance.  These include:

  • Establish qualified provider to enhance care coordination
  • Establish protocols to optimize resource utilization
  • Review of proposed treatment plans including surgical intervention
  • Evaluation of home and equipment
  • Train and educate the individual and all care providers in self care management and prevention
  • Establish proper self care techniques
  • Establish appropriate body positioning and transferring skills
  • Ensure chronic medical conditions are managed and do not negatively impact wound program and prevention
