I have a couple of comments and/or additions to best practices in WC cost control. First, an employer should utilize post offer pre-placement functional testing to be identify functionally qualified employees for a job. This process is inexpensive and has shown to product significant reductions in injuries and their severity. Second, post injury an employer should utilize MEDICAL Case Management to assist the adjuster in making appropriate treatment and return to work decisions. This allows medically trained professionals to work directly with the treating providers to assist in directing care and preventing unnecessary treatment.

Very interested in your article as we have similar situations in UK. I work with an organisation here set up over 25 years ago, to cover just the situations you alude to; how to cope with cognitive difficulties that appear after a mild TBI, how to manage after leaving hospital following neuro surgety (or diagnosisis of neurological illness)when there appears to be little co ordinated assistance to encourage understanding of memory, planning, concentration, or organisational skill impairment, which all have an impact on skills of daily living, let alone financial implications.
We are now gradually, gaining national recognition and hopefully wider awareness of this huge problem.

Thank you for this great post about corporate wellness initiatives. You included some great tips like getting employees to buy in and measuring success. This is very good information for smaller companies; however, we have found that many larger corporations benefit from using a company like MediFit to initiate and implement their employee wellness programs. Regardless of the means by which an organization gets there, wellness programs are a must to increase productivity and employee happiness.

Best

I can't see obesity being a higher problem than regular employees because once you work out the non-work related injuries you will only deal with what has occurred at work. The problem often is that the claims specialist try to completely fix the obese employee rather than getting the employee back to where they were before they were injured. Assertiveness is needed in the industry where the adjustors let the obese employee know what injury is actually being addressed.

Very instructive case study. I infer that the prescribing doctor was over his head, and welcomed Paradigm's intervention. I hope you can induce the Nov WC conference to includes you in presenting several case studies, some successes and some failures.

Dr. Holt,

I actually read this book about 2.5 years ago and remembered how amazed I was with Dr. Doidge's findings. One phrase that stuck to me - 'Use it or lose it'! So true!

I highly recommend this book.

If you aren't doing so already, Paradigm may want to inquire as to possible blast exposure for IW refrrals who have been discharged from the military within the past 5-7 years.

We have found it very difficult, nay, impossible to find a neuropsych in middle Tennessee to help my severe tbi 33 yr old son. And other Drs. he's seen don't seem to have a grasp on tbi patients. I will save up to buy the linked book and will continue to follow your posts. I found the info. very enlightening.

I often think workers' compensation providers sometimes miss the boat on head trauma. Especially when coupled with previous head trauma, minimal brain injury is often misdiagnosed and missed entirely until late in injury when treatment may be less effective. When I ran a claims unit, we implemented immediate three-point contact on any head strike, along with obtaining a thorough medical history to rule out the possibility of previous head strikes. Early treatment is critical to future cognitive function, in my opinion. Thanks for reminding us.

What a surprise stumbling onto your blog today. I work for a large specialized (TBI & SCI) post-hospital rehab provider in Omaha called QLI. From a provider's perspective, I can attest that it is a breath of fresh air for us to work with an industry leader like Paradigm. The catastrophic injury rehab industry greatly benefits from the quality and experience of your case managers and your focus on research and outcomes that are beneficial to all parties involved. I hope that your blog draws the attention that it deserves - I know I will plan to stay tuned!
